what's wrong with my mount?

I'm having issues with my HEQ5 Pro. I recently converted it to OnstepX using a belt drive system.

phd2's guide assistant reports a polar alignment error of around 400 arc minutes. Could this error be causing the problem, or might the worm drive be defective?
